Job Summary


The MDS Nurse employs independent judgment in the planning, organizing, directing, and evaluating of activities involving both professional and supportive nursing staff engaged in the resident care plan.

This role involves assessing the care provided to each resident and ensuring that care plans are consistently updated. The MDS Nurse takes the lead in the Resident Assessment Instrument (RAI) process and assists in the evaluation of potential admissions. Upon admission, as well as during quarterly and annual assessments, the MDS Nurse conducts thorough resident evaluations.

Collaboration is key, as the MDS Nurse works closely with residents, families, advocates, and interdisciplinary colleagues, including healthcare providers, to ensure ongoing care that enhances the quality of life for each resident.

The RN is also expected to participate in leadership weekend on-call schedule rotations.

When functioning on the floor, the Registered Nurse (RN) is responsible for applying the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to deliver personalized nursing care to patients.

The RN demonstrates proficiency and practices within the full scope of nursing expertise, adhering to appropriate age and population-specific standards as designated in their clinical setting.

The RN provides care for residents across all phases of preventive care, health maintenance, diagnosis, and treatment. Responsibilities include coordinating care, conducting resident assessments, providing education, and implementing various nursing interventions.


In a charge nurse capacity, the RN delegates appropriate tasks to both licensed and unlicensed assistive personnel, ensuring that all actions align with job descriptions, skills, resident care needs, and the competence level of the delegate.

As a charge nurse, the RN offers adequate support and guidance to delegates, ensuring the safe and reliable administration of resident care and addressing any care and service concerns.


The RN operates within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in their state of practice and licensure.

The Professional Nursing Practice acknowledges the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as established by the American Nurses Association, serving as the foundation for nursing care delivery and professional conduct.

Qualifications


A graduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited nursing program is preferred.

A Bachelor’s Degree in nursing is also preferred.

At least one year of experience working as a licensed nurse with the senior population is preferred. Specific services or positions may have additional education, training, or experience requirements. Certification as a Resident Assessment Coordinator (RAC-CT), along with management or supervisory experience, is preferred.


The candidate must currently hold an unencumbered RN license with the State Board of Nursing where practice occurs and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.

It is essential to obtain and maintain required certifications and complete necessary continuing education units (CEUs) and competency training. Affiliation with an external professional nursing organization is preferred.
