Job Summary

The Case Manager RN is responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. This role involves coordinating efforts within the rehabilitation team and collaborating with the rehabilitation program director and facility case management director.

Key Responsibilities
  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Minimum of 3 years in nursing practice, with clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, and resourcefulness in navigating community resources.
  • Preferred experience: previous 5 years in nursing practice and 3 years care management, community or ambulatory care management experience, primary care clinical experience, experience with inpatient discharge planning work, Motivational Interviewing, and Intrinsic Health Coaching.
  • Required education: Bachelor of Science in Nursing or bachelor's degree in a related field. If RN, effective 3/1/18, Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position. Previous signed BSN commitment letter OR grandfathering will be accepted between departments and Mission Health entities. Exemptions may be approved with CNO System Council approval.
  • Preferred education: Master's in Nursing.
  • Required license: If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON.
  • Required certification: BLS Healthcare Provider.
  • Preferred certification: Level I: maintains nationally recognized certification in Case Management, such as ACM, CCM, or ANA certification.
