Hospice Nurse Specialist

**Job Summary:**
The Hospice On-Call Nurse will partner with our local and regional leadership as well as the Interdisciplinary Teams to administer patient care as needed while on-call.
**Key Responsibilities:**
• Provide high-quality patient care, including comprehensive OASIS assessments and care plan development.
• Conduct home visits, administer medication, and perform assigned duties, including wound care, treatments, and procedures.
• Practice knowledge of Medicare CoP's, proficient documentation skills, attention to detail, prioritization, and time management.
• Evaluate effectiveness of services provided to the patient, consulting with physicians as needed.
• Ensure accurate and timely charting in the Electronic Medical Record.
• Monitor clients' conditions, reporting changes to Clinical Manager and Physicians.
• Follow up with, executing, and properly documenting doctors' orders.
• Case management and care coordination while in the field with support from the clinical manager.
• Maintain compliance with Federal and State guidelines.
**Requirements:**
• Registered nurse with current licensure to practice professional nursing in the state of practice.
• Demonstrated ability to supervise and direct professional administrative personnel.
• Experience in case management, disease management, home health care nursing, hospital nursing or intensive out-patient education.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
**Skills Required:**
• Organized and resourceful self-starter, strong ability to work in a team.
• Excellent observation, verbal, written and interpersonal communication skills.
• Ability to cope with the mental and emotional stress of the position.
**Work Environment:**
• Work in patient(s) homes as well as agency office.
• Drive in own car to and from patients' home.
