Key Responsibilities:
- Provide skilled nursing interventions to patients with various medical conditions, including chronic illnesses and post-surgical care.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
- Collaborate with patients, family members, and caregivers to educate them on disease management, medication adherence, and self-care techniques.
- Document patient progress and outcomes in a timely and accurate manner, ensuring compliance with regulatory requirements and company policies.

Requirements
• Licensed as a Practical Nurse in the State of Florida
• One year of clinical experience as an LPN preferred
• One year of experience in the specialty of home health preferred
