We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities- Evaluate patients' speech and language abilities and develop personalized treatment plans
- Direct and provide speech/language pathology services to patients, including assessment, diagnosis, and treatment of communication disorders
- Collaborate with healthcare teams to ensure comprehensive care and coordination
- Provide education and training to patients and their families on effective communication strategies and techniques
- Document patient progress and maintain accurate records
- Master's degree in Speech Language Pathology or related field
- Current certification in Speech Language Pathology (CCC-SLP) or equivalent
- Minimum of 6 months of experience as a Speech Language Pathologist
- Home health experience preferred
- Current CPR certification
- Good organizational and communication skills
