Medical Care Provider for Seniors

About the Role
As a seasoned nurse practitioner, you will provide high-quality primary care to seniors and patients with complex needs in various settings. Your responsibilities will include performing evidence-based medical care, collaborating with healthcare professionals, and maintaining accurate patient records.
Key Responsibilities
- Plan, direct, and evaluate patient medical needs
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Ensure patient medical records are kept up to date
Requirements
To be successful in this role, you must have an active state Nurse Practitioners license, current National Board Certification, and DEA licensure. A Master's degree in Nursing is also required, along with five or more years of nursing experience and at least two years of nurse practitioner experience. Strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment are also essential.

Salary and Benefits
The estimated salary for this role is $100,000 - $130,000 per year, with competitive benefits including weekdays and daytime shifts only, mileage reimbursement, flexible schedule, employee referral program, professional licensure reimbursement, tuition reimbursement, paid time off, 401(k) retirement savings plan, medical, dental, and vision insurance, life insurance, Flexible Spending Account (FSA), employee discounts, CME paid time off for SNF-based providers, and CME cost reimbursement program.
