As a Home Health Physical Therapist, you will be responsible for helping patients maintain, restore, and improve movement, activity, and function. Your duties will include:
- Providing interventions and conducting reexaminations every 30 days
- Modifying interventions as necessary and performing discharge planning
- Ongoing assessment and documentation of patient progress
To be successful in this role, you must have:
- A current license to practice physical therapy specific to the state you are assigned to work in
- Graduation from an accredited physical therapy educational program
- At least one year of related physical therapy experience (approval required for less than one year of experience)
- Current CPR certification
