Job Description

As a Home Health Physical Therapist, you will be responsible for helping patients maintain, restore, and improve movement, activity, and function. Your duties will include:

  • Providing interventions and conducting reexaminations every 30 days
  • Modifying interventions as necessary and performing discharge planning
  • Ongoing assessment and documentation of patient progress
Requirements

To be successful in this role, you must have:

  • A current license to practice physical therapy specific to the state you are assigned to work in
  • Graduation from an accredited physical therapy educational program
  • At least one year of related physical therapy experience (approval required for less than one year of experience)
  • Current CPR certification
