Job Title: Registered Nurse - Home Health

BAYADA Home Health Care is seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality home health care to our clients in Downingtown, Pennsylvania.

Job Description:
  • Provide comprehensive home health care services to clients in their homes, including wound care, medication management, and education on self-care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop plans to address any identified gaps in care.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
  • Document all interactions with clients, including assessments, interventions, and outcomes, in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices in home health care.
Requirements:
  • Current licensure as a Registered Nurse in Pennsylvania.
  • Minimum of one year of recent, verifiable clinical experience.
  • Prior home care experience preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal and communication skills.
  • Solid computer skills and experience with electronic medical records (EMR) preferred.
