Job Title: LPN II - Clinical Care Specialist

About the Job:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Clinical Care Specialist. This is a dynamic role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.

Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse or Physician
  • Assess patient needs, develop care plans, and implement interventions to promote positive outcomes
  • Collaborate with interdisciplinary teams to coordinate patient care and achieve treatment goals
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
  • Document patient information accurately and thoroughly in electronic medical records
  • Participate in quality improvement initiatives to enhance patient safety and satisfaction

Requirements:

  • Diploma or equivalent in nursing from an accredited program
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice
  • Minimum 2 years of experience in pediatric nursing
  • Strong clinical skills, including vital sign measurement, medication administration, and wound care
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment with multiple priorities
