Job Title: Dialysis Registered Nurse, Peritoneal RN

We're seeking a skilled Dialysis Registered Nurse, Peritoneal RN to join our team. As a key member of our Home Therapy program, you'll provide patient training and ongoing support for patients choosing a home dialysis modality.

Key Responsibilities:
  • Train patients and/or care partners in self-care Peritoneal Dialysis or Hemodialysis, ensuring compliance with federal and state regulations.
  • Conduct home visits to assess the patient's home environment and evaluate patient performance.
  • Communicate results of patient assessment and ongoing monitoring to the physician, team members, and others as appropriate.
  • Document all nursing services in the Electronic Medical Record, ensuring accurate reflection of patient status and nursing interventions.
  • Participate in infection control monitoring, implementation, and recording as requested.
  • Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions.
  • Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance.
Requirements:
  • Knowledge of federal, state, local laws and regulations, including health care professionals practice act requirements.
  • Ability to work independently and as part of a team.
  • Excellent communication and documentation skills.
  • Flexibility with staffing locations and hours to accommodate patient and USRC home program needs.
