Independent Optometrist

As an Independent Optometrist leasing space within Walmart stores, you will play a leading role in patient care, focusing on eye and vision health, as well as general well-being.

Key responsibilities include:

1. Establishing daily practice schedules
2. Defining hours of operation
3. Managing patient schedules
4. Setting fee structures
5. Retaining 100% of exam/professional fees

As an independent lease holder, you will enjoy:

To be eligible for this position, you must possess:

1. A Doctor of Optometry (OD) degree from an accredited institution
2. A valid state optometry license
3. An Ophthalmologist (MD or DO) degree from an accredited school
4. A valid state ophthalmology license
5. A General Liability Certificate
6. Professional Liability Insurance Certificate
7. A valid W-9
