Job Title: Care Manager - Long Term Services

We are seeking a skilled Care Manager to join our team at Spectraforce Technologies. As a Care Manager, you will be responsible for managing a caseload of healthcare members with long-term care needs. Your primary goal will be to provide exceptional care and support to these individuals, ensuring their health and well-being are met.

The ideal candidate will have a strong background in healthcare, with experience in care management, home health, or discharge planning. A valid driver's license is required, as you will need to travel extensively to visit patients in the Pasco county/New Port Richey area. Responsibilities:
  • • Manage a caseload of healthcare members with long-term care needs
    • Conduct monthly and quarterly member contact

    • Authorizing and coordinating referrals for services

    • Ensuring provider services are delivered without gaps and identifying functional deficiencies in plans of care

    • Collaborating with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services

    • Assisting members with filing and resolving complaints and appeals


Requirements:
  • • Bachelor's degree or Registered Nurse License and 2+ years of care management experience, Licensed Practical Nurse, and 4+ years of care management experience, or 6+ years of care management experience
    • Home health, discharge planning, or long-term care experience preferred

    • Valid driver's license


Salary: The estimated salary for this position is $60,000 - $80,000 per year, depending on experience.
