Pediatric Pharmacist

Job Summary:
The University of North Carolina Hospital Department of Pharmacy is seeking a highly motivated individual to join our team as a Pediatric Clinical Pharmacist. This role will provide comprehensive pharmaceutical care to patients in our Level IV Neonatal Critical Care Center (NCCC) and other pediatric services.
Responsibilities:
- Ensures timely medication access for service patients, including order review and verification.
- Clarifies incomplete, inaccurate, or contraindicated prescriptions with the prescriber, nurse, or other health professional, as appropriate.
- Identifies and intervenes on problems related to the medication needs of the patient.
- Utilizes and analyzes patient data to provide optimal drug therapy recommendations.
- Assesses the patient's medication therapy regimen and makes appropriate clinical interventions with the provider.
- Performs protocol-driven clinical services, such as IV to oral dosing, stewardship activities, and therapeutic dose monitoring.
- Compounds and oversees the preparation and dispensing of medications, including sterile products.
- Maintains patient profiles within the electronic medical record to facilitate communication.
- Complies with the medication formulary and therapeutic substitution programs.
- Facilitates discharge and transfer of patients, including medication reconciliation and discharge counseling.
- Practices medication safety, including adverse drug event and medication error reporting.
- Participates in Code Blue situations for pediatric patients.
- Precepts students, residents, and participates in training new pharmacy staff.
Requirements:
- PharmD or BS Pharmacy Degree
- Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment
- Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience
