Minds Matter, LLC is seeking a highly skilled Physical Therapist to join our team. As a Physical Therapist, you will play a crucial role in helping individuals with brain injuries live independently at home.
Job SummaryThe successful candidate will be responsible for providing rehabilitation services to individuals with brain injuries, with a focus on improving their motor functioning and mobility. You will work closely with consumers to develop skilled therapy goals and provide supervision to physical therapy assistants.
Responsibilities and Duties- Implement Minds Matter, LLC's philosophical principles in all aspects of service delivery.
- Review and provide feedback on daily notes of assistant therapists.
- Assist individuals in identifying their needs and developing appropriate compensation techniques.
- Perform evaluations to address consumers' physical and mental abilities.
- Complete documentation as necessary, including evaluations, progress notes, and daily notes.
- Integrate employment, educational, and independent living goals into therapy.
- Collaborate with consumers and other professional staff to ensure goals are being met.
- Attend team meetings and assist consumers in goal setting.
- Provide services only as designated on each individual consumer's authorized plan of care.
- Assess and make recommendations for adaptive equipment and home modifications.
- Work directly with consumers to improve skills in all aspects of their life goals.
- Educate and train families and caregivers about brain injury strategies and techniques.
- Attend all trainings and workshops to maintain up-to-date skills and knowledge.
- Establish and maintain a professional rapport with consumers, co-workers, and other stakeholders.
- Promote public relations throughout the agency and community.
- Demonstrate a willingness to serve on agency and interagency task groups.
- Perform other duties as assigned by supervisors.
- Maintain confidentiality of protected health information in accordance with HIPAA regulations.
- Ability to teach skills to individuals with disabilities.
- Effective supervision and mentoring of therapy assistants.
- High degree of sensitivity towards all people.
- Ability to deal professionally with individuals on a one-to-one basis.
- Excellent time management skills.
- Creativity.
- Excellent verbal and written communication skills.
- Ability to meet deadlines.
- Basic to moderate technology skills.
- Successful completion of a physical therapy program from an accredited university.
- Current licensure in the state of Kansas.
- Some lifting required for consumers with physical needs.
- Good verbal skills and listening ability.
- Ability to read and write.
- Basic iPad proficiency.
- Reliable transportation.
- Daily check of voicemail and email.
