Job Title: Intake RN

At Universal Health Services, we are seeking a highly skilled and compassionate Intake RN to join our team. As a key member of our behavioral health team, you will be responsible for assessing the medical and behavioral needs of patients and assisting in the process of responding to referrals and other requests for services and information.

Responsibilities:
  • Conduct and document the complete nursing assessment, including an admission summary and nutrition assessment
  • Identify, document, and prioritize mental/medical problems, including the need for detoxification as appropriate
  • Obtain admission orders from physicians and complete nursing admission notes
  • Transcribe and note doctor's orders
  • Initiate the initial multidisciplinary treatment plan, noting initial diagnosis, psychiatric/psychosocial problem, and obtain patient's signature if applicable
  • Inventory patient's home medication with patient's signature and notify house supervisor to retrieve for patient lock-up until discharged
  • Initiate close observation (CO) forms
  • Document patient information and nursing care in the provision of nursing services
  • Report any signs or symptoms of infection in patients to the infection control nurse
  • Verify the accuracy of patient records
  • Employ principles of communication, interviewing techniques, problem-solving, and crisis intervention when performing psychotherapeutic interventions
  • Utilize appropriate interventions in psychiatric and medical emergencies
  • Prioritize and formulate an initial plan of care based on patient assessment and according to policy
  • Collaborate with other team members
  • Act as the patient's advocate and assure that patient rights are upheld
  • Comply with established measures for infection control, OSHA regulations, and risk management
  • Utilize time constructively and in an organized manner to accomplish assigned responsibilities
Requirements:
  • Graduate from an accredited program of professional nursing required; bachelor's degree preferred
  • A minimum of three (3) years' experience in a psychiatric healthcare facility preferred
  • Currently licensed to practice by the California State Board of Nursing
  • A working knowledge of the DSM-IVR and the Mental Status Exam
  • A working knowledge of the Addiction and Recovery field
  • CPR certification and successful completion of CPI (Crisis Prevention Intervention) training
Working Conditions:

The Intake RN will work in a fast-paced environment, providing direct patient care and completing non-direct duties. The ability to sit and stand, walk, reach, kneel, bend, stoop, turn, twist, and throw (occasionally to frequently) is required. The ability to see well enough to read charts, treatment plans, and typewritten materials is also required. The Intake RN will be exposed to chemicals, dust, mist, and steam in the housekeeping tasks and will be required to work at a rapid pace and perform a variety of repetitive duties.

The Intake RN will be responsible for assessing, diagnosing, and treating a variety of patient populations, including children, adolescents, adults, and senior populations. The ability to communicate effectively with patients and coworkers, adhere to safety policies and procedures, and maintain confidentiality of information is required.

The Intake RN will work as a team player, demonstrating tact, resourcefulness, patience, and dedication. The ability to accept direction and adhere to policies and procedures is also required.

The Intake RN will be responsible for maintaining all statistics, providing support to the Outcome Study, and other data as assigned. The ability to establish and maintain effective relationships with customers, practicing good standards of customer service, is required.

The Intake RN will be responsible for working with children, adolescents, adults, and senior populations, and must be familiar with common diagnoses and specific assessment interviewing skills for each population group.

The Intake RN will be responsible for 85% of work time providing direct patient care, including assessment, diagnosing, and direct treatment intervention, and the remaining 15% of work time completing non-direct duties, such as clinical supervision and clinical documentation.
