Key Responsibilities- Provide clinical expertise in nursing management, documentation, and care planning
- Process start of care workflow and maintain accurate records
- Participate in Interdisciplinary Group (IDG) meetings and contribute to quality assurance initiatives
- Review and adjust visit plans as necessary, ensuring seamless transitions for patients
- Manage admissions, deaths, and discharges, ensuring proper documentation and compliance
- Assist with data collection for compliance, quality assurance, and staff development
- Communicate effectively with referral sources to ensure smooth admissions processes
- Associate degree in Nursing with current licensure
- Three years of practice as a Registered Nurse
- Previous Hospice experience preferred
- Knowledge of the Hospice Philosophy of Care
- Proficiency in Microsoft Word, Excel, Outlook, and other software programs
