Job Title: Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Trinity Springs, a 150-unit senior living community in Oxford, FL.

About the Role:

As a Licensed Practical Nurse, you will be responsible for providing high-quality clinical care and services to residents in our assisted living and memory care communities. You will work closely with our interdisciplinary team to ensure that our residents receive the best possible care and support.

Responsibilities:
  • Provide direct care to residents, including administering medications, monitoring vital signs, and providing wound care.
  • Develop and implement individualized care plans for residents, in collaboration with our healthcare team.
  • Monitor and report any changes in a resident's condition to the Clinical Manager and attending physicians.
  • Document resident care using computerized medication administration and charting software.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
Requirements:
  • Current Florida LPN license and good standing.
  • Experience in Assisted Living or Memory Care preferred.
  • Knowledge of FL assisted living AHCA regulations.
  • Strong communication and organizational skills.
  • Self-starter with a desire to be part of a team.
