Job Title: Travel Nurse RN Home Health

Tact Staff is seeking a skilled Travel Nurse RN Home Health professional to join our team for a travel nursing assignment in Medford, New York.

The ideal candidate will have a minimum of 1 full year of adult home health experience or 1 full year of hospice experience, depending on the specialty. The contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. The average number of patients per day is 5-8, with points broken down as follows: 30 points per week. Each type of visit and activity (documentation, travel, etc.) holds a different point value.

The contractor will be required to have reliable transportation, a current driver's license, and agency-required liability insurance. The mileage reimbursement will be based on the IRS standard rate, and the contractor will be compensated from patient home to patient home. The commute to and from the first and last patient is not billable.

The job requires proficiency in HomeCare HomeBase (HCHB) and Oasis, with the ability to work independently and as part of a team. The contractor will be responsible for coordinating and directing the provision of home nursing care according to physician's orders, based on agency policies and procedures.

Key Responsibilities:

  • Coordinate and direct the provision of home nursing care according to physician's orders
  • Responsible for the competent application of the nursing process
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses

Requirements:

  • Minimum 1 full year of adult home health experience or 1 full year of hospice experience
  • Current driver's license and agency-required liability insurance
  • Reliable transportation
  • Proficiency in HomeCare HomeBase (HCHB) and Oasis
