Job Title: Patient Care Manager RN

Job Summary:
We are seeking a skilled and compassionate Patient Care Manager RN to join our team at LHC Group. As a Patient Care Manager, you will play a vital role in providing high-quality patient care and leading our clinical team to deliver exceptional results.

Key Responsibilities:
  • Supervise and coordinate clinical services to ensure seamless patient care.
  • Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
  • Coordinate patient referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Review assessments and plans of care daily, per assigned workflow, and consult with clinicians with recommendations as appropriate.

Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
