Position Overview
We are seeking a Certified Nursing Assistant who will conduct home health visits for our established agency. This role is ideal for individuals with strong clinical abilities and outstanding communication skills, committed to delivering excellent patient care.
Key Responsibilities
- Deliver personal care services based on patient assignments and promptly respond to their needs.
- Ensure patient safety by using equipment correctly and meeting their safety requirements.
- Provide personal hygiene assistance, including bathing, oral care, and changing bed linens as required.
- Support patients with dressing and undressing as needed.
- Assist with feeding patients as directed.
- Monitor and document vital signs, including temperature, pulse, respiration, and blood pressure, as per orders.
- Observe and care for patients' skin to prevent tissue breakdown.
- Report any significant changes in the patient's condition to the supervising Registered Nurse.
- Facilitate patient mobility and exercise as instructed by healthcare professionals.
- Assist with toileting needs and light laundry tasks as assigned.
- Perform range of motion exercises and other basic procedures as part of therapy services.
- Provide respite care for patients and their families when on-site.
- Maintain cleanliness in the patient's living area as directed.
- Adhere to documentation standards and professional conduct guidelines.
- Engage in quality improvement initiatives and team activities.
- Respect the personal needs and environment of patients and their families.
- Participate in on-call duties as assigned.
- Fulfill state training requirements.
