Job Summary

We are seeking a highly skilled and dedicated Habilitation Services Coordinator to join our team at AHRC Suffolk. As a key member of our staff, you will play a vital role in coordinating and overseeing the habilitation services for individuals with intellectual disabilities.

Key Responsibilities:

  • Coordinate and implement program plans, ensuring that individuals receive meaningful and varied activities.
  • Provide mentoring, support, and in-classroom supervision to DSP staff.
  • Act as liaison between home and program, communicating needs and updates.
  • Organize and conduct team meetings, ensuring effective communication and collaboration.
  • Complete monthly notes in Medisked, adhering to OPWDD regulations.
  • Run Medisked reports to ensure timely completion of daily data.
  • Attend scheduled Coordinator meetings, staying informed and engaged.
  • Work with Care Manager to schedule annual and semi-annual meetings, ensuring necessary assessments are completed.
  • Write the Staff Action Plan (SAP) or Habilitation Plan in Medisked, based on individual needs.
  • Complete annual/semi-annual documentation, forwarding to Day Services Information Specialist.
  • Review finalized SAP and supports with staff, ensuring proper implementation.
  • Ensure completed SAP/supports are distributed prior to effective dates.
  • Update records in Medisked as needed, ensuring accuracy and completeness.
  • Participate in daily program and community integration activities.

Requirements:

  • Bachelor's degree and a minimum of 1 year experience working with intellectually disabled individuals, or High School Diploma/GED plus 2 years experience and demonstrated excellent communication skills.
  • Readiness to learn and utilize relevant agency computer applications.
