Job Summary

We are seeking a skilled Hospice Clinical Social Worker to join our team in the Boston area. As a member of our team, you will have the opportunity to work with patients and their families, providing emotional support and guidance throughout their healthcare journey.

Key Responsibilities
  • Provide clinical social work services to patients and their families, including assessment, counseling, and advocacy
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure comprehensive and coordinated care
  • Participate in interdisciplinary team meetings to discuss patient progress and develop care plans
  • Document patient interactions and progress in accordance with company policies and procedures
Requirements
  • Master's degree in Social Work or related field
  • Valid license to practice as a clinical social worker in the state of Massachusetts
  • Experience working in hospice care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
