Patient Care Partner

We are seeking a skilled Patient Care Partner to join our Float Pool team on a part-time basis. As a Patient Care Partner, you will work collaboratively with our patient care team to deliver high-quality care to our patients. This role requires a strong commitment to patient safety, dignity, and well-being.
Key Responsibilities- Participate in the delivery of patient care under the direction of an RN/LPN, including but not limited to:
- Lifting, turning, and positioning patients using Safe Patient Handling equipment (over 35 pounds), as appropriate.
- Observing confused and difficult patients.
- Assisting with range of motion exercises.
- Mobilizing patients - transfers and ambulation, utilizing Safe Patient Handling equipment (over 35 pounds), as appropriate and following the Mobility protocol.
- Administering cleansing enemas.
- Removing peripheral IV catheters.
- Applying external catheters or incontinence devices.
- Sitting with confused/disoriented patients or those requiring 1:1 observation for safety/suicide purposes, as assigned.
- Performing and recording accurately:
- Temperature, pulse, respirations, blood pressure, heights, and weights.
- I & O.
- Records bowel movements.
- ADLs and activities.
- Performing and recording the following specimen collection:
- Obtaining urine, stool, and sputum specimens for patients; instructing patients in proper specimen collection technique.
- Completing EKGs.
- Glucose monitoring in Non-Critical Care Areas.
- Conducting bladder scanning.
- Basic ostomy care.
- Transporting patients as needed.
- Serving, setting up, and retrieving trays.
- Distributing water pitchers as appropriate.
- Ordering and distributing nourishment.
- Transporting equipment.
- Transporting blood products to and from the patient care area.
- Participating in patient safety/patient satisfaction.
- Answering call bells.
- Participating in patient rounding.
- Reporting any signs of abuse to the nursing staff.
- Recognizing, troubleshooting, and initiating corrective action needed on equipment.
- Maintaining a neat and tidy environment (empties laundry, delivers equipment, keeps patient rooms clean and safe).
- Inventorizing and assuring disposition of patients' belongings when admitted, transferred, and/or discharged.
- Assuring proper storage of equipment.
- Recognizing emergency situations and initiating a plan of action.
- Notifying RN/LPN of any changes seen in patient's condition.
- Complying with policies and procedures of the hospital/nursing department.
- Supporting the philosophy of the hospital and department of nursing.
- Maintaining CPR certification and practice.
- Demonstrating cost-effective patient care by demonstrating proper use and care of equipment, appropriate and prudent use of supplies, accurate charging of supplies, performing other division-specific tasks, and appropriate utilization of available resources.
- Participating in performance improvement activities to improve service and care. Demonstrating strong communication and organizational skills.
- Ability to communicate using telephones, computer systems.
- Answering telephone promptly and politely, identifying self, title, and department. Receiving and sending messages in an accurate and timely fashion.
- Communicating with patients, family, and members of the healthcare team in a concise, tactful, and considerate manner. Must represent the hospital in a professional courteous manner, while being sensitive to how others perceive both verbal and non-verbal communications.
- Assisting in the orientation of new personnel and serving as a role model to other employees.
- Demonstrating willingness to accept non-routine work assignments as appropriate.
- Encouraged to participate in community activities.
- Attending and participating in unit council (70% attendance).
