Key Responsibilities
  • Develop and implement individualized plans of care for patients, taking into account their medical history, current health status, and personal preferences.
  • Conduct regular assessments and evaluations to ensure that patients are receiving the most effective and efficient care possible.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure that patients receive comprehensive and coordinated care.
  • Provide education and support to patients and their families, empowering them to take an active role in their care and make informed decisions about their health.
  • Supervise and delegate tasks to certified nursing assistants and licensed practical nurses, ensuring that patients receive the highest level of care possible.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in patient care.
Requirements
  • Current RN license in the state of Colorado.
  • Minimum of one to two years of experience in home health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to lift, push, and pull a minimum of 50 pounds.
