About the Role

We are seeking a dedicated and compassionate individual to join our team as a Mental Health Rehabilitation Worker at Fresh Start Intensive Residential Treatment Services (IRTS), Prairie Community Services, Inc. (PCS), in Duluth, MN.

Key Responsibilities
  • Provide care and services to individuals with intellectual disabilities, brain injuries, and/or mental illness
  • Support individuals in achieving their goals and improving their quality of life
  • Collaborate with the treatment team to develop and implement individualized care plans
  • Assist with daily living activities, such as personal care and household chores
  • Document progress and provide regular updates to the treatment team
Requirements
  • Must be 21 years of age and have a high school diploma or equivalent
  • Must meet company driving criteria and be able to pass a state background study
  • Has a high school diploma or equivalent
  • Holds an associate of arts degree in one of the behavioral sciences or human services, or is a registered nurse without a bachelor's degree, or who within the last ten years has:
    • Three years of personal life experience with serious and persistent mental illness
    • Three years of life experience as a primary caregiver to an adult with serious mental illness or traumatic brain injury
    • 4000 hours of supervised paid work experience in the delivery of mental health services to adults with serious mental illness or traumatic brain injury
