CHRISTUS Health is seeking a highly skilled Pharmacist Clinician to join our team in Anticoagulation Management. As a key member of our healthcare team, you will be responsible for assessing, planning, delivering, coordinating, teaching, and providing patient disease management care/treatment and department services.
Key Responsibilities- Assess patient condition and develop individualized treatment plans
- Provide education and counseling to patients and families on disease management and treatment options
- Collaborate with healthcare teams to ensure seamless care coordination
- Conduct physical assessments and order diagnostic tests as needed
- Develop and implement disease management programs to improve patient outcomes
- Graduate of an ACPE accredited School of Pharmacy
- Licensed Registered Pharmacist (RPh) in the State of New Mexico
- Licensed Pharmacist Clinician (PhC) by the New Mexico Board of Pharmacy
- Registered as a Pharmacist Clinician with the Board of Medicine to exercise prescriptive authority
- Certification in Basic Life Support for Healthcare Providers (BLS)
- Advanced training/certification in anticoagulation and/or disease management
- Experience in anticoagulation management and disease management
- Strong communication and interpersonal skills
- Ability to work collaboratively with healthcare teams
May be exposed to physical injury, toxic substances, infectious diseases, and bloodborne pathogens. Requires knowledge of safety standards to limit exposure to infectious diseases.
