Job Title: MDS Coordinator

GPAC is seeking an experienced MDS Coordinator to join our team at a skilled nursing facility. As a key player in our efforts to ensure compliance and optimize reimbursement, you will play a critical role in our facility's success.

Job Summary:

The MDS Coordinator will oversee the completion of MDS assessments in a timely and accurate manner, ensuring compliance with federal and state regulations, as well as facility policies. You will work closely with our interdisciplinary team to develop individualized care plans based on comprehensive resident assessments.

Key Responsibilities:
  • Manage the MDS process, ensuring timely and accurate completion of assessments
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Monitor and audit documentation for accuracy, completeness, and compliance with CMS regulations
  • Train and educate staff on MDS processes, regulations, and documentation requirements
  • Optimize facility reimbursement through effective management of MDS data submission and quality reporting
Qualifications:
  • Licensed RN or LPN in [Insert State]
  • Minimum of 2 years of experience as an MDS Coordinator or in a related role within a long-term care or skilled nursing facility
  • Comprehensive knowledge of MDS 3.0, RAI process, and care planning
  • Strong understanding of Medicare, Medicaid, and reimbursement processes
  • Excellent attention to detail, organizational, and time-management skills
  • Strong interpersonal skills with the ability to communicate effectively across disciplines
