We are seeking a skilled Registered Nurse - RN to join our team at Madison Center in Cheat Lake, WV. As a Care Management Nurse, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to develop effective care plans.
Key Responsibilities- Perform comprehensive nursing assessments and work collaboratively with the nursing team, other disciplines, patients, and families to develop and implement individualized care plans.
- Establish realistic, measurable patient goals and interventions to minimize rehospitalizations and optimize patient outcomes.
- Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- Competitive hourly rate: $37 - $40 per hour.
- Full-time position with a 12-hour shift schedule (Monday, Tuesday, and Wednesday).
