Job Summary: We are seeking a dedicated and compassionate Social Worker to join our team at the University of Minnesota. As a key member of our Department of Family Medicine and Community Health, you will be responsible for providing high-quality patient care and support services to individuals struggling with substance use disorders.

Key Responsibilities:
Complete diagnostic evaluations of patients suffering from substance use disorders (SUD)
Provide applicable care/service adjusting approaches to reflect developmental level of population served
Communicate in a respectful manner and ensure a safe, secure environment
Partner with patient/caregiver/management in care/decision making
Individualize care/service plan to meet patient needs
Modify interventions/service based on population served
Provide patient education based on assessment of learning needs of patients
Provide brief evidence-based counseling to patients
Meet with patients being seen by medical providers within the clinic to provide brief interventions for emotional health of social problems
Perform psychological assessment of factors interfering with care
Work directly with patients to set care goals and help create treatment plans including psychological and mental health needs
Document appropriate information in the patient's medical record to ensure communication of the patient's needs to the treatment team
Coordinate care for patients, including assisting providers and staff in obtaining resources for patients
Serve as an embedded member of an integrated and consultative team to help those suffering from SUD
Care Coordination process for clinical team's patients, including referrals, hospital discharge coordination, working with providers to complete care plans, creating care goals, and follow-up with patients on goal setting
Monitor transportation concerns and other access barriers for patients
Document visit frequency for treatment of SUD and outcomes, including videoconferencing and other telehealth visits
Track no shows and help patients make appointments
Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in optimal recovery outcomes
Work with the clinic's health care team to provide direct patient care through assessment and referrals to appropriate community, governmental, or social service programs
Advocate for patient needs within UMPhysicians organization and the broader community
Assist patients with counseling related to resources and case management for their needs
Assist patients in understanding the healthcare and managed care delivery system and act to resolve obstacles that prevent them from receiving adequate mental health care
Support patients through transitions of care, including hospital to clinic, emergency room to clinic, clinic to treatment
Help develop protocols that help connect patients in early recovery to care coordinators
Panel management of clients to improve follow-up care, including continuity with providers
Document services delivered and identify areas of strength and gaps in care
Help address any care gaps revealed
Engage in population health activities to support risk reduction techniques to increase healthcare access to underserved populations
Work with the team to seek out resources within the community for housing, food support, family support, inpatient treatment, job support, and others
Maintain a calendar of community events and forms, participate in recovery fairs
Update existing and create new information materials to share with partners for distribution
Create, stock, and distribute Fentanyl and Xylazine testing kits, complete with information about getting access to treatment resources
Create and distribute Narcan reversal kits along with education around how and when to use them
Distribute educational materials around overdose prevention and how to access care; revise annually to stay current
Distribute wound care kits (i.e., for injection site infection) to community partners working directly with clients with OUD
Distribute educational materials on how to access care; revise annually to stay current
Attend community events and forums to distribute materials and answer questions
Meet with community groups to better understand access barriers
Distribute informational resources about treatment options at each level of care; revise annually to remain current

Qualifications:
Licensed Social Worker (LICSW, LCSW, LMSW, LGSW) or Licensed Professional Clinical Counselor (LPCC) as defined by MN statute
License must be active and in good standing
BA/BS degree in social work, counseling, or related field, plus at least six (6) years of experience, or an advanced degree plus at least four (4) years of experience
At least two (2) years of experience must be post-licensure
MN state licenses, or eligible to be licensed in the State of Minnesota and eligibility to become a credentialed provider with insurance entities
Excellent verbal and written communication skills
Ability to provide compassionate and effective counseling services
Must be able to thrive in a dynamic, fast-paced environment with frequently shifting priorities, deadlines, and variable workloads

Preferred Qualifications:
Master's degree in social work, counseling, or related field
Experience delivering culturally sensitive care to diverse patient populations and supporting individuals in underserved, lower socioeconomic communities
Experience in providing diagnostic assessments and direct evidence-based clinical services to adults or ability to develop these skills with supervision
Individual support using Motivational Interviewing, Cognitive Behavioral Therapy (CBT) skills, Social skills training, Coping skills training, Relaxation training, Solution-Focused Brief Therapy, Health and behavioral coaching, and Psychoeducation
Community outreach and patient advocacy
Case management related to care for substance use disorders
Experience working in an outpatient clinic setting
