Job Summary

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Home Health Care RN. As a Home Health Care RN, you will provide one-on-one care to patients in the comfort of their own homes.

Responsibilities
  • Provide high-quality patient care and education to patients and their families
  • Collaborate with other healthcare professionals to develop and implement individualized care plans
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement effective care plans that address patients' unique needs
  • Document all patient interactions and care provided
Requirements

To be successful in this role, you must have:

  • A current valid nursing license in the state of New Jersey
  • Graduation from a qualified nursing program
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and ability to learn electronic medical records software
