About the RoleAs a Private Duty Nurse RN, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing continuous care to remain at home with their loved ones. You will work closely with our patients, their families, and our interdisciplinary team to develop and implement individualized care plans that meet their unique needs and goals.
Responsibilities- Provide skilled nursing care to patients in their homes, including administration of medications, wound care, and vital sign monitoring.
- Assess patient needs and develop individualized care plans in collaboration with the patient, family, and interdisciplinary team.
- Implement and update care plans as needed, ensuring that patients receive high-quality, patient-centered care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current RN licensure in the state of practice.
- One year of experience as a Registered Nurse, preferably in home health or a related field.
- Ability to travel to patient homes and work independently with minimal supervision.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
