Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will provide personalized care and support to patients in their homes, helping them to maintain their independence and quality of life.

Key Responsibilities
  • Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC).
  • Assist patients with self-administered medications, ensuring accurate dosing and observation.
  • Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
  • Help patients maintain good personal hygiene, including bathing, grooming, skin care, and other activities.
  • Assist with feeding patients, communicating basic nutrition principles, and observing and recording food and fluid intake.
  • Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
  • Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.
Requirements
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
