Job Overview

Clarvida is seeking a dedicated and compassionate ACT Case Manager to join our team. As a key member of our Assertive Community Treatment (ACT) team, you will work closely with clients to develop and implement individualized service plans, promoting their recovery and independence.

Key Responsibilities:
  • Engage clients in creating and implementing individual service plans, identifying their goals, strengths, and barriers.
  • Link clients to community services and provide support and training on living skills, such as grocery shopping, budgeting, and social skills.
  • Monitor client progress and behaviors, working collaboratively with the ACT team to support goal attainment.
  • Provide transportation as needed and participate in a one-week on-call rotation every six weeks.
Requirements:
  • Bachelor's degree in Psychology, Social Work, or a related field.
  • Experience working with adults with severe and persistent mental illness, preferred.
  • Professional and courteous communication, with knowledge of and sensitivity to culturally diverse and special needs populations.
  • Valid Georgia Driver's License and clean Motor Vehicle Record (MVR).
  • Clean Criminal Background Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation-child or adult).
