Job Summary:We are seeking a highly skilled Geriatric Therapist to join our team. As a Geriatric Therapist, you will be responsible for providing individual, group, and family therapy to our clients. You will conduct initial assessments, intakes, and prepare documentation for client charts. You will also input data into our profiler computerized medical records and prepare treatment plans with clients.
Responsibilities:- Conduct individual, group, and family therapy sessions
- Perform initial assessments and intakes, and prepare documentation for client charts
- Input data into profiler computerized medical records
- Prepare treatment plans with clients, including goals and progress tracking
- Present cases to psychiatrists regarding client medication
- Work closely with Mental Health Technicians to address client needs
- Provide crisis intervention as needed
- Participate in departmental, divisional, and other meetings as required
- Complete billing documentation in a timely and accurate manner
- Conduct follow-up with clients
- Perform other duties as assigned by the Division Director
