Position Overview

The main objective of this role is to deliver hospice care in accordance with physician directives and the patient’s treatment plan, adhering to hospice protocols and procedures through the nursing process. As a Practical Nursing Specialist, you will operate under the guidance of a registered nurse.

Reporting Structure

Reports to: Director of Clinical Services

Collaborates with: Interdisciplinary Team and administrative personnel

Key Responsibilities

  • Upholds the mission and values of Patriot Hospice, emphasizing the importance of human dignity and a respectful approach to individuals facing life-threatening or chronic illnesses.
  • Adheres to the Nursing Standards set forth by Patriot Hospice.
  • Establishes and nurtures positive relationships with potential referral sources.
  • Conducts nursing visits as required, alerting the case manager or physician of any irregular findings.
  • Demonstrates knowledge of hospice policies, as well as state and federal regulations related to hospice and nursing services.
  • Implements nursing care in line with current regulations and guidelines.
  • Provides written and verbal reports to the Director of Clinical Services as necessary regarding the nursing service department's operations.
  • Engages in the hospice quality assurance program as directed by the Director of Clinical Services.
  • Fosters effective collaboration with contracted facility nursing staff to ensure seamless integration of hospice care into the facility’s care plan for patients.
  • Ensures thorough documentation of patient care services to support accurate professional billing.
  • Performs administrative tasks including completing medical forms, reports, evaluations, and charting as needed.

Qualifications

Must hold a degree from an accredited nursing school and possess a valid nursing license in the state of South Carolina.

Essential Skills and Competencies

  • Proficient in reading, writing, speaking, and understanding the English language.
  • Able to make independent decisions when necessary.
  • Capable of interacting tactfully with personnel, patients, families, visitors, and government representatives.
  • Knowledgeable about hospice nursing practices, medical procedures, and relevant laws and regulations.
  • Possesses leadership qualities and the ability to work collaboratively with and supervise other staff members.
  • Able to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures essential for delivering quality care.
  • Exhibits patience and tact, especially when dealing with challenging patients.
  • Capable of conveying information regarding a patient’s condition effectively.
  • Does not pose a direct threat to the health or safety of others in the workplace.
  • Possesses effective computer skills for documentation purposes.
  • Knowledgeable about HIPAA regulations and compliance.

Work Environment

  • Requires travel within the service area using a personal vehicle; must have a valid driver’s license and auto insurance.
  • Works in various settings including office environments, community locations, and long-term care facilities.
  • Engages in physical activity throughout working hours.
  • May experience frequent interruptions and interact with patients, staff, visitors, and government representatives under various conditions.
  • May encounter emotionally charged situations with patients and families.
  • Communicates with medical staff, nursing personnel, and other department supervisors regularly.
  • May work beyond standard hours, including weekends and holidays, as needed.
  • May be called back during emergencies (e.g., severe weather, evacuations).
  • Participates in community health initiatives as appropriate.
  • Engages in ongoing educational opportunities.
  • May be exposed to risks such as falls, burns, and infectious diseases.
  • May need to handle hazardous materials and chemicals.
  • Must be able to lift and move a minimum of 25 pounds to a height of at least 5 feet and carry such weight for a distance of 50 feet.
