Job Summary

We are seeking a highly skilled LPN Licensed Practical Nurse to join our team at American House Senior Living Communities. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents, promoting their independence and well-being.

Key Responsibilities
  • Administer medication and provide assistance with daily living activities, ensuring the highest level of care and comfort for our residents.
  • Conduct regular assessments and evaluations to identify changes in resident status, needs, or preferences, and communicate these findings to the Wellness Director and/or physician as necessary.
  • Develop and implement individualized care plans to meet the unique needs of each resident, promoting their physical, emotional, and social well-being.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication among staff members.
  • Provide education and support to residents and their families on health-related topics, promoting informed decision-making and empowerment.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance the quality of care provided.
Requirements
  • Current Unencumbered LPN license in the state
  • 2+ years of nursing experience, preferably in long-term care or geriatrics
  • Up-to-date first aid and CPR certification
  • Knowledge of the physiology and psychology of older adults
  • Exceptional communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members
Preferred Qualifications
  • Experience working with dementia residents
  • Knowledge of geriatric care principles and practices
  • Ability to work flexible hours, including evenings and weekends
