We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to patients in their homes, delivering high-quality nursing care and support.
Job Summary:The Private Duty Nurse RN is responsible for providing skilled nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. This is a rewarding opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with our interdisciplinary team.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current, unrestricted state license as a Registered Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have a strong sense of smell and touch.
- Must be able to sufficiently reposition patients and move equipment without assistance.
