- Develop and Implement Discharge Plans: Create and implement discharge plans that address patients' psychosocial needs, ensuring a smooth transition to post-acute care.
- Provide Education and Support: Educate patients and families on health-related issues, provide emotional support, and connect them with community resources.
- Document Patient Information: Accurately document patient information, including assessments, treatment plans, and progress notes.
- Master's Degree in Social Work: Possess a Master's degree in Social Work from an accredited institution.
- State Licensure: Hold a valid Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license in the State of Texas.
- Experience: New graduates with hospital internship experience preferred; one year of hospital social work experience also preferred.
- Effective Communication: Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Community Resource Knowledge: Have knowledge of community resources and healthcare financial and payer issues, with the ability to utilize alternative resources to fill gaps in established community resources.
- Computer Skills: Be proficient in Microsoft Office Suite, including Access, Excel, Outlook, PowerPoint, and Word.
