Job Title: Travel Nurse RN Case Manager

Tact Staff is seeking a skilled Travel Nurse RN Case Manager to join our team in Gainesville, Florida.

Job Summary:

We are looking for a highly motivated and experienced Travel Nurse RN Case Manager to work in our Acute Care Case Management department. The ideal candidate will have a strong background in case management and be able to work independently in a fast-paced environment.

Responsibilities:
  • Provide case management services to patients in the Acute Care department
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Communicate effectively with patients, families, and healthcare team members to ensure optimal care outcomes
Requirements:
  • Current RN licensure in the state of Florida
  • Minimum 2 years of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical records and other relevant software
Work Environment:

The Travel Nurse RN Case Manager will work in a fast-paced Acute Care department with a team of experienced healthcare professionals. The ideal candidate will be able to work effectively in a dynamic environment and prioritize tasks to meet deadlines.
