Job Summary

The Nurse Home Visitor is a pivotal role in transforming the lives of vulnerable babies, mothers, and families by providing comprehensive nursing services and implementing the Nurse-Family Partnership model.

Key Responsibilities
  • Provide personalized nursing services to first-time expectant parents during pregnancy and until the child is two years old.
  • Conduct home visits and community-based assessments to identify and address health and wellness needs.
  • Develop and implement individualized care plans to promote healthy outcomes and empower parents to make informed decisions.
  • Collaborate with community resources and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing training and professional development to maintain expertise in maternal child health and community nursing.
  • Travel to visit families, attend training sessions, and engage in reflective supervision with site supervisors.
Requirements
  • Registered Nurse (RN) with a Bachelor's degree in Nursing (BSN) and current licensure in Delaware.
  • Minimum of 2 years of experience in public health, community, and/or maternal-child health nursing.
  • Experience with labor and delivery, home visiting, and bi-lingual communication (English/Spanish) is highly desirable.
  • Current CPR certification and valid driver's license required.
