About the Position:
We are seeking a highly skilled and experienced Patient Care Professional to join our multidisciplinary team in the Department of Psychiatry at our MidHudson Regional Hospital in Poughkeepsie, New York. The ideal candidate will have a proven track record of clinical accomplishments in the treatment of patients with serious mental illness, and will be responsible for providing high-quality patient care services, including triage, diagnostic evaluations, follow-up care, prescribing, and individual counseling and group treatment.
Responsibilities:
- Providing patient-centered care to adults with serious mental illness, including triage, diagnostic evaluations, and follow-up care.
- Conducting diagnostic evaluations and developing treatment plans to address patients' mental health needs.
- Prescribing and managing medications to ensure optimal patient outcomes.
- Collaborating with multidisciplinary teams to provide comprehensive care and support to patients and their families.
- Maintaining accurate and up-to-date records of patients' medication and treatment plans.
