OverviewWe are seeking an exceptional Family Health Advocate and Community Outreach Specialist to join our team. This role plays a vital part in delivering high-quality services to pregnant women, infants, and young children and their families. As a key member of our team, you will be responsible for providing direct client services, assessing individual needs, and developing personalized plans of care.
Primary Responsibilities
- Provide comprehensive home visitation services to families, including education, screenings, and referrals.
- Conduct thorough assessments of family needs, identify goals, and develop individualized plans of care.
- Promote healthy pregnancy outcomes, infant growth, and development through access to healthcare services and community resources.
- Foster strong relationships with clients, parents, and guardians by teaching positive parenting skills, promoting parent-child interaction, and administering educational home visit lessons.
- Maintain accurate records of client activities, progress notes, and termination summaries, adhering to contractual requirements and CHS standards.
- Bachelor's Degree in Social Work, Health Education, or a comparable human services field required.
- Two years of experience working with children and families; previous experience in a social-services organization preferred.
- Florida Driver's License within 30 days from hire, with daily access to a reliable and insured vehicle required.
- Equivalent combination of education and experience may be considered.
The ideal candidate will possess excellent communication, collaboration, and project management skills, as well as the ability to work autonomously and prioritize tasks effectively. Proficiency in computer systems, MS Office, and presentation platforms is also essential.
