Job Title: Chronic Care Nurse

This is a work-from-home position for individuals residing in states with lenient regulations, allowing employees to maintain a balance between personal and professional life.

Position Overview:

We've transformed remote care management at Signallamp Health to ensure chronically ill patients receive timely support. Our team of nurses maintains consistent connections with patients, offering guidance on navigating health challenges and bridging gaps in traditional healthcare.

Nurses play a vital role in our patient-centric approach, working closely with providers and in-office staff to deliver seamless care experiences.

Key Responsibilities:
  • Engage in regular check-ins with patients, discussing condition changes, follow-up on appointments, and understanding new instructions from the patient's provider.
  • Manage a caseload of 200-250 chronically ill patients, prioritizing their health and well-being.
  • Ensure timely follow-up by recognizing when additional communication or intervention is required to address ongoing patient issues.
  • Bond with patients, understanding their unique personalities and challenges, and establish trust with new patients and their families.
  • Act as a vital link within the patient's care team, communicating seamlessly with providers and in-office staff.
  • Guide patients in prioritizing their health and understanding their conditions, advocating for their needs and addressing medical concerns promptly.
  • Utilize technology to manage and coordinate care, gathering resources for continuity of care and accurately documenting actions taken for patients using Electronic Medical Record (EMR) systems.
Qualifications:
  • Active, unrestricted LVN, LPN, or RN license.
  • Graduate of an accredited LVN, LPN, or RN program.
  • 4 years of clinical experience, including 2 years in home health, primary care, or long-term care.
  • Ability to work a full-time schedule, Monday through Friday, from 8-4:30 p.m. EST or 11 a.m.-7:30 p.m. EST, with a minimum upload speed of 12 mbps and download speed of 20 mbps.
  • Proficient using Electronic Medical Records (EMR), with excellent verbal and written communication skills, strong organizational skills, and attention to detail.

The estimated salary for this position ranges from $60,000 to $80,000 per year.
