About the Role

Bristol Hospice is seeking a highly skilled and compassionate Advanced Practice Registered Nurse (APRN) to join our team as a Nurse Practitioner in Hospice Care. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and families in need of end-of-life support.

Key Responsibilities
  • Provide Comprehensive Hospice Care: Evaluate and assess patient health history, health promotion, and diagnosing medical conditions; manage health problems by directing and developing the plan of care, prescribing medications or treatments, and coordinating and collaborating with patients, families, and other healthcare practitioners.
  • Collaborate with Interdisciplinary Team: Work closely with the hospice medical director, attending physician, and other healthcare practitioners to create, review, and revise patient plans of care.
  • Communicate Effectively: Communicate with patients, families, and other healthcare practitioners to coordinate the plan of care, provide education and counseling, and ensure continuity of care.
  • Maintain Accurate Records: Complete, maintain, and submit accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes, and other documentation in the medical record.
  • Stay Up-to-Date with Industry Developments: Maintain knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services, as well as industry best practices.
Requirements
  • Education and Experience: Master's degree in Nursing with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Certification and Licensure: Current nursing licensure in the state and accreditation from an approved certifying body for advanced practice nursing as required by the state.
  • Skills and Abilities: Strong communication and interpersonal skills, ability to work effectively in a team environment, and proficiency in electronic medical records.
