Position Overview

The Pharmacist will report directly to the Pharmacy Manager and will be involved in delivering exceptional pharmaceutical care.

As a vital member of the Pharmacy team, you will be responsible for providing comprehensive pharmaceutical care to patients within our service area. We are committed to supporting your professional development and facilitating career advancement opportunities.

This position is PRN, with a need for flexibility to work Onsite Monday through Friday, including occasional Saturdays for on-call duties.

Key Responsibilities

As a Pharmacist, your duties will include:

  • Assessing and overseeing drug therapy to meet the comprehensive needs of home care patients in collaboration with healthcare professionals.
  • Complying with organizational policies and all relevant state and federal regulations governing pharmacy practice.
  • Participating in quality assurance initiatives and adhering to accreditation standards.
  • Coordinating new patient referrals and admissions through collaboration with nursing staff and customer service.
  • Reviewing and verifying all new patient orders and documenting them accurately.
  • Assisting healthcare professionals in planning and implementing patient care in home settings.
  • Establishing therapeutic goals for drug therapy and monitoring safety and efficacy.
  • Conducting thorough assessments of patient, drug, and disease information to identify potential drug-related issues.
  • Monitoring drug therapy outcomes by analyzing laboratory and clinical data.
  • Providing expert consultation and addressing inquiries from patients, families, and healthcare professionals regarding medications.
  • Maintaining detailed documentation of all clinical activities and communications.
  • Processing and verifying all medications and supplies prior to dispensing.
  • Keeping abreast of advancements in pharmacotherapy through continuous education and professional development.

Qualifications Required

  • Bachelor of Science in Pharmacy is required; Doctor of Pharmacy is preferred.
  • Active Registered Pharmacist License in the state(s) of practice.
  • A minimum of two years of recent experience in an acute-care environment or one year in home infusion therapy.
  • Experience in compounding infused medications is essential.
  • Proven track record in evaluating and managing drug therapies for home care patients.
  • Strong organizational and time-management skills.
  • Membership in professional organizations is advantageous.
  • Excellent verbal and written communication abilities.
  • Flexibility to work on-call during evenings and weekends as needed.
