Job Title: Hospice Registered Nurse

At Liberty Hospice, we are seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families facing terminal illness.

Job Summary:

We are looking for a compassionate and skilled Hospice Registered Nurse to provide patient-centered care and support to our patients and their families. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a team environment.

Responsibilities:
  • Provide high-quality patient care and support to individuals and their families facing terminal illness.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on end-of-life care and related topics.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
  • Knowledge of the framework, function, and organization of a home health and hospice care program.
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
