The Clinical Therapist - Acute Care will be a key member of our multidisciplinary team, responsible for providing direct clinical services to patients and their families. The primary focus will be on completing comprehensive psychosocial assessments, developing and implementing individualized treatment plans, and facilitating psycho-educational training and family meetings.
Key Responsibilities:- Conduct thorough psychosocial assessments and develop initial discharge plans.
- Develop and implement treatment plans for patients based on assessment, coordinating additional services as needed and revising plans as necessary.
- Conduct individual, group, or family sessions as appropriate for the treatment plan.
- Apply psychotherapeutic techniques and interventions to treat diagnosed emotional and behavioral disorders.
- Participate in multi-disciplinary treatment planning conferences, presenting patient information and recommending effective treatment interventions.
- Maintain a caseload as assigned and develop and implement special patient groups or program modules.
- Prepare timely progress notes and treatment updates in the medical record, ensuring all clinical documentation meets regulatory and accrediting standards.
- Ensure each assigned patient has a structured and clinically appropriate post-hospitalization care plan, acting as a liaison to referral sources and supporting continuation of care.
- Provide crisis intervention to patients and families in acute distress and introduce and coordinate community resources, including financial, housing, legal, social, recreational, occupational, spiritual, and support groups.
- Perform case management, discharge, or aftercare planning as needed.
- Master's Degree in Social Work, Counseling, or Psychology required.
- At least one year of experience as a therapist with all age groups.
