As a Senior Community Care Professional at Optum Home & Community Care, you will play a vital role in our team providing care to patients at home, in nursing homes, assisted living facilities, and senior housing. Your primary responsibilities include delivering cost-effective, quality care to assigned members, managing chronic and acute conditions, and ensuring diagnoses are accurately coded and documented.

You will coordinate care as members transition through different levels of care and care settings. Your goal is to monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change. You'll also evaluate the effectiveness, necessity, and efficiency of the plan, making revisions as needed.

The ideal candidate has 1+ years of hands-on post-grad experience within Long Term Care, understanding of Geriatrics and Chronic Illness, and proficiency in developing and maintaining positive customer relationships. You should be adaptable to change and able to work independently with minimal supervision. The estimated salary for this role is $42.31 to $83.27 per hour.

* Deliver cost-effective, quality care to assigned members
* Manage both medical and behavioral, chronic and acute conditions effectively, and in collaboration with a physician or specialty provider
* Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
