Gentiva Hospice is seeking a skilled Nurse Practitioner to join our team in providing high-quality care to patients and their families. As a Hospice Nurse Practitioner, you will play a vital role in delivering compassionate and personalized care to those in need.
Key Responsibilities- Conduct Face-to-Face (F2F) encounters to gather clinical information and support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Perform physical assessments within regulatory time frames established by CMS.
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists, to the physician.
- Make recommendations to the IDG based on F2F assessment findings.
- Notify the hospice agency when unable to complete a F2F visit according to schedule.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
