This role is crucial in ensuring that patients are making progress towards their health goals by continuously evaluating and monitoring their care.
Key Responsibilities
- Collaborates with healthcare professionals to assess, plan, and implement individualized patient care plans.
- Coordinates discharge planning by liaising with appropriate agencies and ensuring a smooth transition for patients.
- Develops and maintains personalized care coordination strategies tailored to each patient's needs.
- Acts as a resource for physicians and hospital staff regarding available community services and facilities to aid in discharge processes.
- Completes necessary documentation in compliance with federal, state, and local regulations.
- Ensures adherence to quality care standards by monitoring and reporting potential risks and quality management issues.
- Conducts regular case reviews to identify delays or discrepancies in patient care and addresses them with relevant departments.
- Maintains accurate data collection in accordance with departmental policies.
- Communicates effectively with physicians regarding any issues that require resolution or intervention.
- Utilizes established medical necessity criteria to verify that patients are receiving appropriate care in the correct setting.
- Engages with payer liaison to address any cases that may not meet medical necessity guidelines.
- Works collaboratively with payer liaison to resolve barriers and facilitate timely discharges or alternative care plans.
- Stays informed about concurrent denials through ongoing communication with payer liaison.
Work Environment
This position is primarily conducted in a clinical setting. The individual is responsible for fulfilling all job-specific duties and adhering to organizational policies and procedures.
Position Details
This role encompasses responsibilities in the Emergency Room.
Work Schedule: 40 hours per week; includes days, evenings, and weekends.
On-Call: None required.
Experience: A minimum of 3 years of nursing experience is essential.

Education Requirements
